What if I lose my card?
If you have lost the card issued to you from the Waterbury Campus, you must fill out a replacement ID form. There is a $5 replacement fee. Forms are available at the following Waterbury offices: F 205, F 129 or F 204A at the Waterbury Campus. You may also download the form here. A replacement card will be available in 14 days.
